As a patient of Your Best Life Medical eClinic, PLLC, you have the right to:

Receive reasonable communication assistance when needed. We will make reasonable efforts to provide appropriate auxiliary aids and services for patients with hearing, vision, speech, language, or other communication needs.
Because services are provided by telehealth, some accommodations may require advance planning. Please tell us as early as possible about any communication or accessibility needs.

Participation in Your Care
Participate actively in decisions about your care, including discussing treatment options, asking questions, requesting clarification, and expressing preferences regarding your care plan.
Informed Consent and Refusal of Care
Give informed consent before treatment when required and refuse recommended evaluation, treatment, medication, testing, or referral. If you decline recommended care, we will explain the potential health risks when appropriate.
You do not have the right to demand services, prescriptions, tests, referrals, or treatments that the provider determines are medically unnecessary, clinically inappropriate, unsafe, outside the practice’s scope, or not suitable for telehealth.
Choice of Surrogate Decision-Maker
Identify a person to make healthcare decisions for you if you become unable to make decisions for yourself, consistent with applicable law.
Advance Directives
Create an advance directive, living will, or healthcare power of attorney. You may provide a copy to the practice for inclusion in your medical record.
Know Your Care Team
Know the name, credentials, and professional role of the clinician providing your care.
Telehealth Information
Receive information about the nature of telehealth services, including its potential benefits, limitations, privacy considerations, and alternatives. You may withdraw consent to telehealth services at any time, although this may limit the practice’s ability to continue providing care.
Personal Privacy During Telehealth Visits
Receive telehealth care in a manner designed to protect your privacy. You are encouraged to participate from a private, safe location and to tell your provider if anyone else is present during your visit.
Referrals and Higher-Level Care
Receive guidance or referral for in-person evaluation, emergency care, specialty care, diagnostic testing, or other services when your needs cannot be safely or appropriately managed through telehealth.
Financial Information
Receive information about expected practice charges, payment responsibilities, and available self-pay, superbill, financing, or out-of-network benefit options before non-emergency services are provided, when reasonably possible.

Voice concerns, complaints, or recommendations without fear of discrimination, retaliation, coercion, or interruption of medically appropriate care.

Patient Responsibilities
As a patient, you are responsible for:
Providing Accurate Information
Provide complete, accurate, and current information about your symptoms, medical history, medications, allergies, past surgeries, hospitalizations, diagnoses, pregnancy status when relevant, and other information needed for safe care.
You are also responsible for promptly updating your address, telephone number, email address, pharmacy, emergency contact, and insurance or payment information.
Participating Honestly in Telehealth Visits
Participate in telehealth visits from a location where you are physically located in Virginia at the time of the appointment, unless otherwise approved by the practice and permitted by law.
You are responsible for providing your current physical location and a call-back number at the beginning of each telehealth appointment in case emergency assistance is needed.
Maintaining a Safe Telehealth Environment
Use a private, safe setting for appointments whenever possible. Do not participate in a telehealth visit while driving, operating machinery, or in another situation that could compromise safety or privacy.
Asking Questions
Ask questions when you do not understand your diagnosis, treatment plan, medication instructions, follow-up recommendations, financial responsibilities, or the limitations of telehealth care.
Following the Care Plan
Follow the agreed-upon care plan, including medication instructions, recommended testing, referrals, monitoring, follow-up appointments, and instructions to seek in-person or emergency care when advised.
If you cannot or do not plan to follow a recommendation, you are responsible for notifying the practice so that risks and alternatives can be discussed.
Seeking Emergency Care When Needed
Call 911 or go to the nearest emergency department for emergency symptoms, including severe chest pain, severe trouble breathing, signs of stroke, severe allergic reaction, loss of consciousness, thoughts of harming yourself or others, or other potentially life-threatening concerns.
Your Best Life Medical eClinic does not provide emergency services and is not a substitute for emergency care.
Respectful Conduct
Treat providers, staff, and other individuals interacting with the practice respectfully. Threatening, abusive, discriminatory, sexually inappropriate, harassing, or disruptive behavior may result in termination of the patient-provider relationship, consistent with applicable law and patient-safety obligations.
Protecting Your Portal and Account Information
Keep your patient portal login credentials private and notify the practice promptly if you believe your account has been accessed without authorization.
Financial Responsibility
Pay applicable visit fees, balances, copayments, charges for approved services, and any fees disclosed in the practice’s Financial Policy. You are responsible for understanding that a superbill or out-of-network claim submission does not guarantee insurance reimbursement.
Providing Advance Directives
Provide the practice with a copy of your advance directive, healthcare power of attorney, or other relevant legal healthcare documents if you would like them included in your medical record.
Important Telehealth Limitation
Telehealth may not be appropriate for every health concern. The provider may determine that an in-person examination, urgent care visit, emergency evaluation, diagnostic testing, or specialist referral is necessary. You are responsible for following these recommendations to help protect your health and safety.

Price Transparency
At Your Best Life Medical eClinic, we believe patients should understand the expected cost of care before services are provided. We aim to make pricing clear, straightforward, and easy to review so you can make informed decisions about your health care.
Our practice is primarily cash-pay and serves adults age 18 and older in Virginia through telehealth. We do not bill insurance directly. However, eligible patients may be able to use HSA/FSA funds or seek possible out-of-network reimbursement from their insurance plan using a superbill.
How Our Prices Are Determined
Our prices are based on the type of visit, time and clinical complexity involved, and services reasonably expected to be needed for your care.
Your total cost may include:
Telehealth visit fees
Follow-up visit fees
Medical weight-management or wellness program fees
Optional membership fees
Diagnostic review fees, when applicable
Laboratory testing, imaging, prescriptions, or other services ordered through outside providers
Some services may require additional evaluation, testing, treatment, or follow-up that cannot be fully anticipated before your visit. We will discuss recommended services and expected costs with you whenever reasonably possible before they are ordered.
What Is Included in Your Visit Fee
Your visit fee generally includes the telehealth evaluation, medical decision-making, treatment recommendations, and prescriptions when clinically appropriate.
Your visit fee does not generally include:
Laboratory testing or specimen-collection fees
Imaging studies
Prescription medication costs
Pharmacy charges
Specialist consultations
Emergency department, urgent care, hospital, or in-person examination costs
Fees charged by independent laboratories, imaging centers, pharmacies, specialists, or other third parties
Outside providers and facilities set their own prices and billing policies. Your Best Life Medical eClinic does not control those charges.
Insurance, HSA/FSA, and Out-of-Network Benefits
Your Best Life Medical eClinic is a cash-pay practice and does not submit claims directly to insurance companies.
After payment, patients may request a superbill for eligible services. A superbill is an itemized receipt that may be submitted to your insurance plan for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends on your specific plan, deductible, coinsurance, coverage rules, and out-of-network benefits.
Please contact your insurance company before your visit to ask about:
Whether telehealth visits are covered
Whether nurse practitioner services are eligible for reimbursement
Your out-of-network deductible and coinsurance
Whether a superbill is required
Whether the specific service you are seeking is covered
HSA/FSA cards may be accepted for eligible services. Patients are responsible for confirming whether a charge qualifies under their individual HSA/FSA plan.
Laboratory, Imaging, and Prescription Costs
If laboratory testing, imaging, medications, or referrals are recommended, these services may result in separate charges from outside providers.
Whenever possible, we will discuss lower-cost options, self-pay options, and appropriate alternatives. However, final charges for outside services are determined by the laboratory, imaging facility, pharmacy, specialist, or other provider delivering that service.
Patients are responsible for confirming costs and insurance coverage with outside providers before receiving services.
Good Faith Estimates
You have the right to receive a Good Faith Estimate explaining how much your medical care is expected to cost.
Under federal law, patients who do not have insurance or who choose not to use insurance for care may request a Good Faith Estimate of expected charges before services are provided.
You may request a Good Faith Estimate at any time by contacting our office. For scheduled services, estimates are generally provided when required by law.
A Good Faith Estimate is an estimate only and does not include charges from outside providers, pharmacies, laboratories, imaging centers, specialists, hospitals, or other facilities unless specifically listed.

Payment Responsibility
Payment is due at the time services are scheduled or provided, unless another written arrangement has been made in advance.
Patients are responsible for all charges associated with services provided by Your Best Life Medical eClinic and for any charges from outside laboratories, pharmacies, imaging centers, specialists, hospitals, or other providers.
Prices, services, and available programs may change. We will make reasonable efforts to keep current pricing available on our website and discuss applicable fees before services are provided.

Public Records and Information Requests
Your Best Life Medical eClinic, PLLC is a privately owned medical practice and is not a public body subject to the Virginia Freedom of Information Act (FOIA).
Accordingly, the Practice is not required to provide records in response to FOIA requests. Medical records, billing information, business records, personnel information, and other Practice records are maintained and disclosed only as permitted or required by applicable law.
Medical Record Requests
Patients may request access to or copies of their own medical records in accordance with applicable federal and Virginia law.
To request your medical records, please submit your request through the patient portal or contact the Practice directly. We may require verification of identity and a written authorization before releasing records.
Medical records will not be released to family members, employers, attorneys, insurers, or other third parties without a valid written authorization from the patient or another legal basis for disclosure.
